    Summary

Total labyrinth sequestration is rare in purulent epitympanitis and cholesteatoma. A lip mucous membrane flap effectively stops labyrinthectomy-induced liquorrhea, with advanced imaging aiding diagnosis.

    Area of Science:

    • Otolaryngology
    • Neurosurgery
    • Radiology

    Background:

    • Purulent epitympanitis and cholesteatoma can lead to rare cases of total labyrinth sequestration.
    • Diagnosing labyrinth destruction requires advanced imaging techniques.
    • Middle and inner ear inflammation can cause progressive labyrinth damage.

    Observation:

    • Two case observations of total labyrinth sequestration were documented.
    • A novel method using a lip mucous membrane flap was employed to manage labyrinthectomy-induced liquorrhea.
    • Combined computer tomography and traditional tomography proved informative for diagnosing labyrinth destruction.

    Findings:

    • The lip mucous membrane flap technique efficiently arrested labyrinthectomy-induced liquorrhea.
    • Combined tomography is effective for diagnosing labyrinth destruction.
    • Functional body changes, such as pregnancy, may stimulate labyrinth destruction during chronic otorrhea and cholesteatoma progression.
    • The vestibular apparatus demonstrates significant compensatory potential even with complete vestibule and semicircular canal destruction due to chronic inflammation.

    Implications:

    • This study offers a practical solution for managing a rare but serious complication of ear surgery.
    • Advanced imaging modalities enhance diagnostic accuracy for labyrinthine pathologies.
    • Understanding the influence of systemic factors on labyrinthine destruction is crucial for patient management.
    • The vestibular system's resilience highlights potential for functional recovery despite extensive damage.
