Osteosarcomas (OS) of the jaws are uncommon, presenting later than skeletal OS and often with pain and swelling. The mixed subtype was most common in gnathic OS cases.

Background:

  • Osteosarcomas (OS) of the jaws are rare, accounting for 6-8% of all skeletal OS.
  • This study investigated gnathic OS characteristics compared to non-gnathic OS across four major treatment centers.

Purpose of the Study:

  • To assess the demographics, clinical presentation, histopathological findings, and prevalence of gnathic OS.
  • To compare gnathic OS with non-gnathic OS in terms of age, site, and clinical manifestations.

Main Methods:

  • A retrospective analysis of 98 OS patients, including 13 with gnathic OS, treated between 1996-2007.
  • Evaluation of age, gender, tumor site, clinical signs/symptoms, grade, and subtype using various staining methods.

Main Results:

  • Gnathic OS (13.2%) presented later (avg. 27 years) than non-gnathic OS (avg. 15 years).
  • Common sites for gnathic OS were the mandibular body and posterior maxillary alveolar ridge, with symptoms including pain, swelling, and tooth loosening.
  • The mixed subtype was the most frequent gnathic OS subtype, and 15.3% of gnathic OS patients died within 10 years.

Conclusions:

  • The prevalence of jaw OS in this study was higher than previously reported.
  • Pain and swelling are key indicators for gnathic OS, which typically manifests in patients aged 20-30.
  • The mixed subtype is the predominant histological subtype of gnathic OS.
