Summary

Ventricular shunts can migrate, but a rare case involved a distal catheter extruding through the mouth after 15 years and three revisions. This highlights the potential for delayed and unusual shunt complications.

  • The patient presented with the unusual symptom of per-oral extrusion of the original distal shunt catheter.
  • This event occurred many years after the initial shunt placement and subsequent revisions.

Findings:

  • The distal catheter from the patient's original ventriculoperitoneal shunt migrated significantly.
  • The migration resulted in the catheter protruding through the oral cavity.
  • This represents an extremely rare late complication of ventriculoperitoneal shunt placement.

Implications:

  • This case underscores the importance of considering rare migration patterns in shunt complications.
  • It highlights the need for vigilance in long-term follow-up of patients with shunted hydrocephalus.
  • Further investigation into the biomechanics and risk factors for such extreme late-stage migration may be warranted.
