Background:

  • Subconjunctival hemorrhage (SCH) is a common condition with unclear pathogenesis.
  • Conjunctivochalasis (CCh), a condition characterized by excess conjunctival tissue, has not been previously studied in relation to SCH.

Purpose of the Study:

  • To investigate the relationship between conjunctivochalasis (CCh) and subconjunctival hemorrhage (SCH).
  • To compare the severity of CCh in patients with SCH versus control subjects.

Main Methods:

  • A prospective, nonrandomized study involving 104 patients with SCH and 120 age- and gender-matched controls.
  • Assessment of CCh grade across 8 conjunctival areas, with specific focus on nasal, middle, and temporal locations.
  • Evaluation of CCh changes related to gaze and digital pressure, and the incidence of superficial punctate keratitis.

Main Results:

  • Patients with SCH exhibited significantly higher mean CCh grades in nasal, middle, and temporal areas compared to controls (P<0.00001 for all).
  • Increased CCh changes with downward gaze or digital pressure and a higher frequency of superficial punctate keratitis were observed in SCH patients (P<0.00001, P<0.00001, P=0.00106).
  • The extent and location of SCH positively correlated with CCh severity (P<0.05).

Conclusions:

  • This study provides the first evidence linking CCh severity to SCH.
  • The findings suggest that conjunctivochalasis may be a significant factor in the pathogenesis of subconjunctival hemorrhage.
