  • Forty implants were placed in 25 patients, with 20 immediately loaded and 20 conventionally loaded.
  • Marginal bone levels were measured from the fixture-abutment junction to mesial and distal bone levels using specialized software.

    • At 1-year follow-up, no significant difference in marginal bone loss was observed between immediate and conventional loading in maxillary implants (p = 0.1031).
    • Slight significant differences were found in mandibular implants, with lower marginal bone loss in the immediately loaded group (p = 0.0141).
    • Overall, crestal bone maintenance was similar between the two loading protocols for modified diameter implants.

    Conclusions:

    • Both immediate and conventional loading protocols demonstrate comparable crestal bone maintenance around modified diameter internal hex implants.
    • The loading protocol may have a slight differential effect in the mandible, favoring immediate loading regarding marginal bone loss.
    • These findings support the use of both loading strategies with modified diameter implants, with careful consideration for mandibular placement.
