Sudden aortic regurgitation in a 54-year-old man was caused by aortic cusp rupture. Mitral valve replacement surgery was successful, with no signs of infection, indicating chronic inflammation and myxomatous changes.

Area of Science:

  • Cardiovascular Surgery
  • Cardiac Pathology

Background:

  • Aortic regurgitation can present suddenly, necessitating urgent surgical intervention.
  • Rupture of an aortic cusp is a rare cause of acute aortic insufficiency.

Observation:

  • A 54-year-old male presented with acute onset aortic regurgitation.
  • Intraoperative findings revealed a detached right coronary aortic cusp prolapsing into the left ventricle.
  • No evidence of bacterial endocarditis was observed.

Findings:

  • Histological examination of the excised aortic cusp showed non-specific chronic inflammatory changes.
  • Focal areas of myxomatous degeneration were also identified in the valve tissue.

Implications:

  • This case highlights a non-infectious etiology for acute aortic cusp rupture.

Related Experiment Videos

  • Understanding the underlying pathology is crucial for appropriate surgical management and prognosis in aortic valve disease.
