Does Robotic Liver Operation Require a Platform-Specific Difficulty Scoring System?

Journal of the American College of Surgeons·2026

Area of Science:

  • Hepatobiliary Surgery
  • Minimally Invasive Surgery
  • Surgical Oncology

Background:

  • Laparoscopic major liver resections present significant challenges for surgeons.
  • This video details a step-by-step laparoscopic technique for left hepatectomy.

Purpose of the Study:

  • To illustrate a standardized laparoscopic technique for left hepatectomy.
  • To evaluate the safety and reproducibility of this minimally invasive liver resection method.

Main Methods:

  • Laparoscopic left hepatectomy with extraparenchymal control of vascular inflow/outflow and hepatic duct division.
  • Eleven procedures performed between 2002-2008 for liver tumors (n=7) and Caroli's disease (n=4).

Main Results:

  • Mean operative time: 248 minutes. Mean blood loss: 129 ml.

  • No intraoperative transfusions or conversions to laparotomy were required.
  • One postoperative biliary collection managed percutaneously; no mortality; mean hospital stay: 7.6 days.
  • Conclusions:

    • The described laparoscopic left hepatectomy technique is safe.
    • The procedure is easily reproducible for selected liver resections.
