Background:

Victims of violence often engage in risky sexual behaviors.
Previous research indicates a correlation between violence and sexual behaviors.

Purpose of the Study:

To investigate the association between coercive first intercourse and unintended first births.
To quantify the risk of unintended first births following coercive sexual experiences.

Main Methods:

Analysis of nationally representative data from the 2002 National Survey of Family Growth.
Inclusion of female respondents aged 18-44 years with reported live births (n=4,136).
Classification of coercion into none/minimal, mild, or significant categories based on self-report.

Main Results:

In 2002, 13.7% of U.S. women (18-44) with live births experienced mild coercion at first intercourse.
9.8% of U.S. women (18-44) with live births experienced significant coercion at first intercourse.
Women experiencing mild (OR: 1.9) or significant coercion (OR: 2.3) had higher odds of unintended first births compared to those with no coercion.

Conclusions:

Coercive first intercourse is significantly associated with unintended first births.
Findings underscore the long-term reproductive health consequences of sexual coercion.
Public health interventions should address sexual coercion to reduce unintended pregnancies.
