Background:

Liver tuberculosis is an uncommon manifestation of extrapulmonary tuberculosis.
Liver tuberculoma represents a rare form of hepatic tuberculosis.
This case highlights liver tuberculoma as a potential occupational disease.

Observation:

A 63-year-old veterinary doctor with liver tuberculosis showed treatment failure after 8 months of anti-tuberculosis drugs.
The tuberculoma relapsed, forming a large abscess (80x65x80 mm) and a satellite nodule (20 mm).

Findings:

Polymerase chain reaction and histology confirmed Mycobacterium tuberculosis complex in the liver.
Surgical resection of three liver segments was performed without complications.
The patient achieved complete recovery three years post-surgery.

Implications:

Liver resection is a viable option for progressive liver tuberculoma unresponsive to medical treatment.
Surgical intervention for liver tuberculoma demonstrates excellent long-term outcomes.
This case underscores the importance of considering surgical management for complex liver tuberculosis cases.
