Background:

  • Retinal arterial macroaneurysms are dilations of retinal arteries.
  • Often associated with hypertension, they can mimic other retinal diseases.

Observation:

  • A hypertensive 59-year-old male presented with sudden, painless vision loss in the left eye.
  • Fundus examination revealed extensive retinal hemorrhage, diagnosed as a retinal arterial macroaneurysm via fluorescein angiography.

Findings:

  • The patient experienced spontaneous resolution of the hemorrhage without intervention.
  • Visual acuity improved from counting fingers to 20/25 within six months.

Implications:

  • Optometrists must recognize the signs and symptoms of retinal arterial macroaneurysms.
  • Awareness of differentials, treatments, and the importance of systemic workup for associated diseases is crucial.
