Background:

Plateau iris syndrome diagnosis often relies on imaging due to clinical ambiguity.
This case highlights a plateau-like iris configuration caused by multiple iridociliary cysts.

Observation:

A patient with elevated intraocular pressure and narrow angles, unresponsive to laser peripheral iridotomy, presented with symptoms suggestive of plateau iris.
Gonioscopy revealed significant iridocorneal contact, prompting further investigation.

Findings:

Ultrasound biomicroscopy (UBM) confirmed a plateau iris configuration secondary to numerous ciliary body cysts.
UBM proved superior to anterior segment OCT in visualizing the ciliary body and confirming the diagnosis.

Implications:

Iridociliary cysts can mimic plateau iris syndrome, leading to worsening symptoms.
UBM is an invaluable tool for diagnosing and managing plateau iris and related conditions.
