Abstract:
In treating erectile dysfunction (ED) effectively, a hierarchy of treatments based on efficacy, safety, and other factors can be a valuable instrument for assisting clinical decisions. First-line therapies, in this hierarchy, include sildenafil--the only orally administered erectogenic drug currently available--vacuum constriction devices, and psychological counseling and sexual therapy. Second-line therapies include intracavernous injection and intraurethral suppositories. Third-line therapies include penile implant surgery. This article presents a broad overview of all available therapies, but concentrates on sildenafil as the current paradigm of broadly efficacious, safe, and convenient treatment for ED.
