Burn Injuries01:22

Burn Injuries

Burn injuries occur when the skin and underlying tissues are damaged due to exposure to heat, electricity, chemicals, radiation, or friction. They can vary in severity, from minor superficial burns to severe deep burns that can be life-threatening.
The damage results in the death of skin cells, which can lead to a massive loss of fluid. Dehydration, electrolyte imbalance, and renal and circulatory failure follow, which can be fatal. Burn patients are treated with intravenous fluids to offset...

Peripheral Artery Disease V: Postoperative Nursing Management

During the postoperative period, it is crucial to focus on maintaining circulation, identifying and managing potential complications, and planning for discharge.Nursing AssessmentVital signs monitoring: Regularly monitor vital signs, including blood pressure, heart rate, respiratory rate, and temperature, to detect early signs of complications such as bleeding and infection.Circulation assessment: Monitor pulses, perform Doppler assessments, and check capillary refill, color, temperature, and...

Summary

Rollover motor vehicle crashes (MVCs) frequently cause severe upper extremity injuries requiring amputation. These injuries, often occurring in summer, are increasing and pose significant risks to patient survival.

Area of Science:

  • Trauma Surgery
  • Orthopedic Surgery
  • Emergency Medicine

Background:

  • Rollover motor vehicle crashes (MVCs) pose a significant risk of upper extremity injury.
  • Mangled extremities with vascular compromise have high amputation rates.
  • Patient positioning during MVCs may affect injury severity.

Purpose of the Study:

  • To describe the care process and outcomes for patients needing emergent upper extremity amputation after rollover MVCs.
  • To analyze patient demographics, injury patterns, and treatment results.

Main Methods:

  • Retrospective analysis of patients undergoing upper extremity amputation following rollover MVCs from 2000-2008.
  • Inclusion criteria: patients requiring emergent upper extremity amputation.
  • Data collected: demographics, injury types, surgical procedures, and outcomes.

Main Results:

  • Seventeen patients underwent upper extremity amputation post-rollover MVC.
  • Injuries predominantly occurred on the side ipsilateral to the occupant (88%).
  • Amputations were common during summer months (76%) and increasing in frequency.
  • Mortality (12%) was linked to traumatic brain injuries.
  • Most amputations (65%) were above the elbow.

Conclusions:

  • Blunt mangled upper extremities requiring amputation are frequently caused by MVC rollovers.
  • The incidence of these injuries is associated with summer and appears to be rising.
  • Prompt surgical intervention is crucial for managing these severe injuries.
