Area of Science:

  • Gastroenterology and Hepatology
  • Transplantation Surgery

Background:

  • Short-gut syndrome (SGS) often necessitates complex nutritional support, sometimes involving liver dysfunction.
  • Combined small bowel and liver transplantation is a complex procedure with limited reported cases.

Observation:

  • A patient with SGS and antithrombin III deficiency underwent combined small bowel and liver grafting.
  • The patient experienced transient, mild graft-versus-host disease and intestinal rejection, which were successfully managed.
  • Parenteral nutrition was discontinued 8 weeks post-transplant.

Findings:

  • The patient achieved normal nutritional indices on an unrestricted oral diet post-transplant.
  • Combined small bowel/liver grafting demonstrated feasibility in this case.
  • The procedure enabled the cessation of parenteral nutrition.

Related Experiment Videos

Implications:

  • Combined small bowel/liver transplantation may be a feasible treatment for select SGS patients with liver disorders.
  • Further research is required to establish the risks, benefits, and broader applicability of this combined transplant approach.
  • This case highlights the potential for improved quality of life through nutritional independence after complex transplantation.
