Summary

Spontaneous bleeding from a scrotal arteriovenous malformation (AVM) is rare. This case highlights the importance of considering AVM in scrotal vascular lesions and planning treatment with specialists.

Background:

Scrotal arteriovenous malformations (AVMs) are exceptionally rare vascular anomalies.
Reported cases are fewer than ten, with potential congenital or post-traumatic origins.

Observation:

A case of a spontaneously bleeding scrotal AVM following remote trauma is detailed.
Differential diagnosis includes varicocele, hemangioma, lymphangioma, and AVM.
Imaging modalities like ultrasound and MRI are crucial for evaluation.

Findings:

Angiography is essential for precise delineation of feeder vessels, vascular takeoffs, and draining veins in scrotal AVMs.
Scrotal AVMs can manifest as scrotal enlargement and a bruit.

Implications:

Accurate diagnosis and characterization of scrotal AVMs are critical.
Multidisciplinary planning involving Interventional Radiology and surgical consultants is necessary for definitive management of these rare lesions.
