Summary

The Mitroflow bioprosthesis shows durability comparable to other aortic valve replacements, with excellent hemodynamics, especially in small aortic roots. It effectively minimizes prosthesis-patient mismatch, improving patient outcomes.

Background:

  • Aortic valve degenerative disease is increasing with life expectancy.
  • There is a need for bioprosthetic aortic valves with superior hemodynamics and durability.
  • The Mitroflow bioprosthesis has a long history but recent US regulatory approval.

Purpose of the Study:

  • To evaluate the durability of the current Mitroflow bioprosthesis.
  • To assess the impact of prosthesis-patient mismatch (PPM) on outcomes.
  • To determine long-term survival and freedom from structural valve deterioration (SVD).

Main Methods:

  • Implantation of Mitroflow bioprosthesis in 381 patients across three centers.
  • Mean patient age of 76.4 years, with a mean follow-up of 5.4 years.
  • Classification of prosthesis-patient mismatch (PPM) using effective orifice area index.

Main Results:

  • 10-year survival rates varied by age group (39.9% for 50-69, 27.0% for 70-79, 16.6% for ≥80).
  • Moderate-to-severe PPM and left ventricular dysfunction were independent predictors of survival.
  • 10-year freedom from SVD was high, comparable to other bioprostheses (e.g., 94.4% for ≥60 years).

Conclusions:

  • The external-mounted Mitroflow pericardial aortic bioprosthesis offers excellent hemodynamics, particularly for small aortic roots.
  • It effectively eliminates moderate and severe prosthesis-patient mismatch.
  • Its durability is comparable to other heterograft bioprostheses.
