Area of Science:

  • Cardiovascular Surgery
  • Interventional Cardiology
  • Medical Devices

Background:

  • Aortic stenosis is a common valvular heart disease, particularly in the elderly.
  • Transcatheter aortic valve implantation (TAVI) has emerged as an alternative to surgical aortic valve replacement (SAVR).
  • Transapical TAVI offers a minimally invasive approach for patients unsuitable for traditional surgery.

Purpose of the Study:

  • To report the initial clinical experience and outcomes of transapical aortic valve implantation.
  • To evaluate the safety and efficacy of this procedure in a high-risk octogenarian population.

Main Methods:

  • Retrospective analysis of six consecutive patients undergoing transapical aortic valve implantation.
  • Assessment of procedural success, intraoperative and postoperative complications.

  • Evaluation of prosthesis function and patient discharge status.
  • Main Results:

    • All six patients were octogenarians with significant comorbidities, carrying an expected mortality of 22% (logistic EuroSCORE).
    • All procedures were technically successful, with no intraoperative or postoperative complications.
    • Five patients were discharged between postoperative days 5 and 7 with normally functioning aortic valve prostheses.

    Conclusions:

    • Transapical aortic valve implantation is a safe and effective procedure in high-risk elderly patients.
    • This approach facilitates successful treatment of aortic stenosis with favorable short-term outcomes and rapid recovery.
