Summary

A retained root tip from a molar tooth mimicked apical periodontitis on dental radiographs. Surgical removal confirmed the root tip, and the patient recovered fully, showing lesion healing.

Background:

  • A retained root tip from a left lower first molar was found near the left lower second premolar apex.
  • Radiographs showed an ill-defined periapical radiolucency with a radiodense component, mimicking apical periodontitis.
  • Differential diagnoses included focal sclerosing osteomyelitis and ossifying fibroma.

Findings:

  • Excisional biopsy revealed the retained root tip with associated granulation tissue.
  • The patient remained asymptomatic with healing of the periradicular lesion after one year.
  • The associated premolar tooth responded normally to vitality testing.

Implications:

  • This case highlights the importance of considering retained root tips in the differential diagnosis of periapical lesions.
  • Accurate radiographic interpretation and biopsy are essential for definitive diagnosis.
  • Conservative management of retained root tips can lead to favorable outcomes.
