Purpose of the Study:

  • To evaluate the outcomes of preemptive kidney transplantation (PKT).
  • To assess the impact of an active PKT policy in an unselected ESRD population.

Main Methods:

  • Analysis of 3400 first kidney transplantations between 1989-2007, with 809 (24%) being PKTs.
  • Comparison of PKT recipients with non-PKT recipients regarding age, donor type, and HLA antibody status.
  • Cox regression analyses for patient mortality and graft failure, considering factors like age, donor type, and HLA compatibility.

Main Results:

  • PKT recipients were younger, had more live donors, and lower panel reactive antibody levels.
  • PKT was associated with a reduced hazard ratio for patient mortality (0.75) and graft failure.
  • In deceased donor (DD) recipients, PKT significantly reduced patient mortality (HR=0.70) and graft failure (HR=0.68).

Conclusions:

  • Preemptive kidney transplantation (PKT) reduces patient mortality and graft failure in deceased donor recipients.
