Summary

Neuroleptic Malignant Syndrome (NMS) developed faster and lasted longer in schizophrenia patients. Consciousness changes were the earliest sign, preceding rigidity and fever, in all patients.

Area of Science:

  • Neurology
  • Psychiatry

Background:

  • Neuroleptic Malignant Syndrome (NMS) is a rare but serious reaction to antipsychotic medications.
  • Understanding NMS presentation and progression is crucial for timely diagnosis and management.

Observation:

  • This case series describes five patients diagnosed with NMS.
  • Schizophrenic patients experienced earlier onset and prolonged resolution of NMS.
  • Deterioration in consciousness was the most consistently early presenting symptom.

Findings:

  • Consciousness decline preceded rigidity and fever in all five patients, suggesting it as a primary diagnostic criterion.
  • Abnormalities of gaze were observed in a significant number of patients.
  • Four out of five patients achieved spontaneous recovery without specific pharmacological interventions.

Implications:

  • Early recognition of altered consciousness may aid in the prompt diagnosis of NMS.
  • The findings suggest that NMS management might not always require specific treatments, with spontaneous recovery being common.
  • Further research into the specific pathophysiology of NMS in schizophrenic patients is warranted.
