Cardiomyopathy VII: Pre and Post Operative Nursing Management

Patients with hypertrophic cardiomyopathy (HCM) and left ventricular outflow tract (LVOT) obstruction who remain symptomatic despite optimal medical therapy may undergo a septal myectomy (Morrow procedure). This procedure involves excising a portion of the hypertrophied septum below the aortic valve using a heart-lung machine to improve blood flow through the LVOT. Effective preoperative and postoperative nursing management ensures successful patient outcomes, minimizes complications, and...

Pericarditis III: Medical Management

The primary objectives of managing pericarditis are to determine the underlying cause, provide effective therapy for treatment and symptom relief, and promptly detect signs and symptoms of cardiac tamponade. The following outlines the essential aspects of medical management for pericarditis:ObjectivesDetermine the Cause: Identifying the underlying cause of pericarditis is crucial for targeted treatment. Causes include viral infections, autoimmune diseases, post-cardiac injury syndrome, and...

Endoscopic Studies II: Thoracocentesis

Thoracentesis(Thoracocentesis), commonly known as pleural tap, is a medical procedure where a 22 gauge needle is inserted into the pleural space, the area between the lung and chest wall. This procedure is commonly performed to diagnose or treat various respiratory disorders.
Description
Excess pleural fluid or air may accumulate in some respiratory disorders in the thoracic cavity. To treat pleural effusion, a physician conducts thoracentesis by carefully piercing the chest wall and entering...

Summary

Heavy silicone oils offer improved inferior retinal tamponade for proliferative vitreoretinopathy. These new agents are well-tolerated and represent a significant advancement in intraocular tamponade technology.

Area of Science:

  • Ophthalmology
  • Retinal Surgery
  • Biomaterials

Context:

  • Inferior proliferative vitreoretinopathy presents significant surgical challenges.
  • Conventional intraocular tamponades have limitations in effectively managing inferior retinal detachments.
  • Development of heavier-than-water tamponades aims to enhance inferior tamponading properties.

Purpose:

  • To define the parameters of an optimal intraocular tamponade.
  • To discuss the physical properties (specific gravity, buoyancy, interfacial tension, viscosity) influencing tamponade performance.
  • To evaluate the clinical efficacy and safety of novel heavy silicone oil tamponades.

Summary:

  • Heavier-than-water intraocular tamponades, including heavy silicone oils like Oxane HD and Densiron 68, demonstrate improved inferior tamponading capabilities.

  • While perfluorocarbon liquids and partially fluorinated alkanes showed emulsification and inflammation risks, admixtures with silicone oil mitigated these issues.
  • Heavy silicone oils are well-tolerated, comparable to conventional silicone oil, and have entered clinical practice.
  • Impact:

    • Heavy silicone oil tamponades represent a new generation of intraocular agents.
    • Improved inferior tamponade can lead to better surgical outcomes for challenging retinal conditions.
    • These advancements offer enhanced therapeutic options for vitreoretinal diseases.
