Purpose of the Study:

  • To investigate diverse potential risk factors for hypospadias, differentiating between distal, middle, and proximal phenotypes.
  • To gain insight into the etiological heterogeneity of hypospadias.

Main Methods:

  • A case-referent study involving 305 boys with hypospadias and 629 controls (middle ear effusion).
  • Parents completed postal questionnaires detailing potential risk factors.
  • Logistic regression analysis was used to estimate adjusted odds ratios (OR) and 95% confidence intervals (CI) for various risk factors, stratified by hypospadias phenotype.

Main Results:

  • General risk factors identified include low birth weight, multiple gestation, maternal diethylstilbestrol exposure history, fertility treatments, paternal subfertility, obesity, and drug use.
  • Familial occurrence of hypospadias showed strong associations with distal (OR 10.4) and middle (OR 9.0) phenotypes, but not proximal (OR 1.8).
  • Low birth weight (OR 9.1) and pre-eclampsia showed a stronger association with proximal hypospadias compared to distal and middle phenotypes.

Conclusions:

  • Hypospadias exhibits etiological heterogeneity, with distinct risk factors influencing different phenotypes.
  • Findings suggest specific mechanisms by which environmental and genetic factors may disrupt normal penile development.
  • Further research into phenotype-specific etiologies is warranted to inform prevention and treatment strategies.
