Autoantibodies to neurofilament (NF) proteins are more common in Down syndrome (DS). Antibodies to NF-H in children with DS may indicate a better prognosis, with HLA-DR15 negatively correlating with NF-H antibodies.

Purpose of the Study:

- Antibodies to dephosphorylated NF-M, NF-M, and NF-L were more prevalent in DS patients compared to controls.
- While overall NF-H antibody frequency was similar, DS patients with mild/moderate disability showed higher prevalence (43.0%) than those with severe disability (14.3%).
- No NF-H antibodies were detected in young children (≤14 years) with severe disability; HLA-DR15 allele showed a negative correlation with NF-H antibodies.
Conclusions:
- The elevated prevalence of neurofilament autoantibodies in DS suggests a complex interplay between neural and immune systems.
- These antibodies might be involved in protecting neural structures from neurodegenerative processes.
- In pediatric DS, the presence of NF-H antibodies could be a marker for a more favorable clinical outcome.
