Summary

Salvage external beam radiotherapy (EBRT) after prostatectomy for rising PSA is curative. This study found that EBRT for detectable PSA after radical prostatectomy has a low risk of late complications.

Background:

  • Radical prostatectomy (RP) is a primary treatment for prostate cancer.
  • A detectable prostate-specific antigen (PSA) level post-RP indicates potential residual disease.
  • Salvage external beam radiotherapy (EBRT) is an option for patients with rising PSA after RP.

Purpose of the Study:

  • To assess the incidence and severity of late toxicities following salvage EBRT in patients with detectable PSA post-RP.
  • To determine the safety profile of salvage EBRT in this specific clinical scenario.

Main Methods:

  • A cohort of 308 patients receiving salvage EBRT for detectable PSA post-RP was analyzed.
  • Treatment involved high-energy photons (6-20 MV) to a median dose of 64.8 Gy.
  • Patients were followed for late toxicity occurring >90 days post-EBRT.

Main Results:

  • Late toxicity was observed in 13% of patients (41/308).
  • Grade 2 urethral strictures occurred in 3.9% of patients.
  • An estimated 0.7% of patients experienced grade 3 or 4 complications by 5 years post-EBRT.

Conclusions:

  • Salvage EBRT is the sole curative treatment for detectable PSA post-RP.
  • This radiotherapy approach can be delivered with a low probability of significant late complications.
