Fractures: Bone Repair

Treatment for a fracture is based on the type of break, the bone affected, and the patient's age.
Minor fractures with no bone displacement are treated by immobilizing the fractured bone using a cast or splint. However, in the case of fractures with displaced bones, the broken bones are repositioned before immobilization to ensure successful healing without deformation and loss of function. The realignment of fractured bone ends is performed through a process called reduction. If the procedure...

Summary

Endosteal implants can cause mandibular fractures, especially in atrophic mandibles. Careful planning, including 3-D imaging and bone grafting, is crucial for prevention and successful treatment of these fractures.

Area of Science:

  • Oral and Maxillofacial Surgery
  • Dental Implantology

Background:

  • Mandibular fractures associated with endosteal implants are a potential complication.
  • Atrophic mandibles present unique challenges for implant placement due to compromised bone quality and reduced blood supply.

Observation:

  • This study reports four cases of mandibular fractures occurring in conjunction with endosteal implant placement.
  • Fractures can occur during or after implant surgery, particularly in severely atrophic mandibles.

Findings:

  • Implant placement in weakened, atrophic mandibles increases the risk of iatrogenic fractures.
  • Maintaining a minimum of cortical bone on labial and lingual aspects post-drilling is essential.
  • 3-D surgical planning is recommended for severe atrophic mandibles to prevent excessive bone reduction.

Implications:

  • Radiological assessment of mandibular anatomy (height, width) is vital before implant insertion.
  • Bone grafting techniques can reinforce the mandible and mitigate fracture risk.
  • Effective treatment of mandibular fractures requires reduction and immobilization for functional restoration.
