Background:

  • Metachromatic leukodystrophy (MLD) is a severe demyelinating disease.
  • Novel therapeutic strategies for MLD are under investigation.
  • Quantifying disease burden is crucial for treatment evaluation.

Purpose of the Study:

  • To develop and validate a quantitative MR imaging scoring system for brain involvement in MLD.
  • To assess the reliability of the proposed scoring method.

Main Methods:

  • Review of 34 brain MR images from 28 MLD patients (late infantile, juvenile, adult).
  • Experienced neuroradiologists and neurologists applied a visual scoring system (0-34 points) based on white matter lesions and atrophy.
  • Readers were blinded to clinical data.

Main Results:

  • High inter-rater reliability (0.988) was achieved with scores differing by a maximum of 2 points.
  • Confluent white matter hyperintensities were observed in 33 of 34 MR images.
  • Serial imaging in 6 patients demonstrated disease progression in 3 and stability in 3, correlating with initial scores.

Conclusions:

  • The developed MR imaging severity scoring method effectively measures brain involvement in MLD.
  • This quantitative tool can aid in monitoring disease progression and treatment response in MLD.
