Disorders of the Female Reproductive System01:24

Disorders of the Female Reproductive System

The female reproductive system can be affected by several disorders, including Premenstrual Syndrome (PMS), Premenstrual Dysphoric Disorder (PMDD), endometriosis, and various forms of cancer. PMS and PMDD are cyclical conditions that cause physical and emotional distress, with symptoms that include edema, mood swings, and food cravings. PMDD is a more severe form of PMS characterized by increased symptom severity that peaks during the luteal phase and tends to improve or resolve shortly after...
Accessory Ducts of the Male Reproductive System01:25

Accessory Ducts of the Male Reproductive System

The male reproductive system is a complex network of organs and tissues that work together to produce and transport sperm. The epididymis, vasa deferens, ejaculatory ducts, and urethra are the accessory ducts involved in sperm maturation and transportation. These ducts play a critical role in the production and transportation of sperm from the testes to the urethra, where it is then released during ejaculation.
The epididymis is a small, comma-shaped organ located at the back of each testicle.
Mitral Valve Prolapse I: Introduction01:27

Mitral Valve Prolapse I: Introduction

IntroductionThe mitral valve, one of the heart's four valves, regulates blood flow. These valves have flaps that open and close to direct blood properly through the heart and body. During each heartbeat, the flaps open for blood to pass through and seal shut to prevent backflow. Specifically, the mitral valve opens to allow blood flow from the heart's upper left chamber to the lower left chamber. It then closes securely as the lower left chamber contracts to pump blood to the body, preventing...
Renal Tubule and Collecting Duct01:24

Renal Tubule and Collecting Duct

The renal tubule is divided into three parts: the proximal convoluted tubule (PCT), the Loop of Henle (LOH), and the distal convoluted tubule (DCT).
Proximal Convoluted Tubule (PCT):
The PCT is the initial segment of the renal tubule, extending from the Bowman's capsule that encloses the glomerulus. Its convoluted structure and microvilli-lined cells increase the surface area for reabsorption. The PCT reabsorbs glucose, amino acids, sodium, and water from the filtrate, ensuring essential...
Development of the Sexual Organs in the Embryo and Fetus01:15

Development of the Sexual Organs in the Embryo and Fetus

Development of the reproductive organs in an embryo starts from a bipotential state. This means the early embryo can develop either male or female reproductive organs. The formation of these organs begins with the growth of gonadal ridges that arise from the intermediate mesoderm during the fifth week of development.
Near the gonadal ridges, two duct systems are present: the mesonephric ducts (Wolffian ducts) and paramesonephric ducts (Müllerian ducts). These ducts form the basis for the male...

Summary

Persistent Müllerian duct syndrome (PMDS) is a rare condition where males have Müllerian duct remnants. This case highlights PMDS discovered incidentally in a cryptorchid male diagnosed with seminoma.

Area of Science:

  • Endocrinology
  • Genetics
  • Reproductive Medicine

Background:

  • Persistent Müllerian duct syndrome (PMDS) is a rare intersex condition.
  • It is characterized by the presence of Müllerian duct remnants in males who are otherwise genotypically and phenotypically normal.
  • PMDS is typically diagnosed in infancy or childhood, but late diagnosis is possible.

Observation:

  • A 40-year-old cryptorchid male presented with a clinical diagnosis of seminoma in his undescended abdominal testis.
  • Abdominal orchidectomy was performed for the suspected seminoma.
  • Histological examination revealed the presence of Müllerian duct structures.

Findings:

  • The histological findings confirmed a diagnosis of Persistent Müllerian duct syndrome (PMDS).

  • This diagnosis was made in adulthood, despite the initial clinical presentation suggesting malignancy.
  • The case underscores the importance of thorough histological evaluation in cryptorchidism.
  • Implications:

    • This case expands the known clinical spectrum of Persistent Müllerian duct syndrome (PMDS).
    • It highlights the possibility of late diagnosis in adult males with undescended testes.
    • Accurate diagnosis of PMDS is crucial for appropriate management and genetic counseling.
