  • Polyalkylimide is a biocompatible, nonresorbable polymer filler used for soft tissue augmentation.
  • Previous literature indicates a low initial complication rate for polyalkylimide treatments.
  • Delayed complications, appearing years post-injection, have been observed.

Purpose of the Study:

  • To comprehensively evaluate complications associated with polyalkylimide treatments.
  • To assess the long-term safety and adverse event profile of polyalkylimide fillers.

Main Methods:

  • A retrospective evaluation of complications was conducted.
  • Data were collected from members of the Dutch Society of Cosmetic Medicine.
  • Complications were analyzed based on patient and treatment rates.

Main Results:

  • A total of 3,196 patients and 4,738 treatments were reviewed.
  • 154 complications were reported, resulting in a 4.8% patient complication rate and a 3.3% treatment complication rate.
  • The most frequent complications included inflammation, hardening, product migration, and accumulation; histological examination via skin biopsy was performed in some cases.

Conclusions:

  • Polyalkylimide treatments can result in complications manifesting years after administration.
  • The observed 4.8% overall complication rate, coupled with the severity and treatment challenges, poses an unacceptable risk for cosmetic procedures.
  • The Dutch Society of Cosmetic Medicine recommends against the use of polyalkylimide for cosmetic purposes.
