Background:

  • Refractory status epilepticus (RSE) carries a poor prognosis with conventional treatments.
  • Prompt seizure cessation is crucial for favorable patient outcomes in RSE.
  • Limited therapeutic options exist for patients with RSE unresponsive to anti-epileptic drugs.

Observation:

  • A series of three consecutive patients with RSE were treated with electroconvulsive therapy (ECT).
  • All patients had failed to respond to standard anti-epileptic drug therapies prior to ECT.
  • ECT was administered as a last resort intervention for persistent seizures.

Findings:

  • Electroconvulsive therapy (ECT) successfully terminated seizures in two out of three patients with refractory status epilepticus.
  • One patient achieved complete neurological recovery following ECT.
  • Another patient experienced mild cognitive impairment and developed epilepsy but regained independent living.

Implications:

  • Electroconvulsive therapy (ECT) shows promise as an effective treatment for refractory status epilepticus.
  • Further clinical investigation is warranted to establish ECT's role in managing RSE.
  • ECT may represent a viable therapeutic option for patients with medically intractable seizures.
