Area of Science:

  • Obstetrics and Gynecology
  • Surgical Outcomes

Background:

  • Peripartum hysterectomy is a life-saving procedure for managing severe postpartum hemorrhage.
  • Understanding risk factors and indications is crucial for prevention and management strategies.

Purpose of the Study:

  • To analyze the incidence, indications, and risk factors of emergency peripartum hysterectomy.
  • To identify key associations that may inform clinical practice and patient counseling.

Main Methods:

  • Retrospective review of 91 cases of peripartum hysterectomy between January 2001 and September 2008.
  • Comparison with randomly selected controls to determine independent risk factors.
  • Statistical analysis including odds ratios and confidence intervals.

  • The incidence of emergency peripartum hysterectomy was 0.67 per 1,000 deliveries.
  • Uterine atony was the primary indication in 57.1% of cases.
  • Multiparity (OR 17.3) and caesarean delivery (OR 6.7) were significant independent risk factors.

Conclusions:

  • Multiparity and caesarean delivery are independently associated with increased risk of peripartum hysterectomy.
  • Uterine atony remains the most frequent indication for this procedure in Turkey.
  • These findings highlight the importance of considering patient parity and delivery mode in assessing hysterectomy risk.
