Summary

Pancreatic islet transplantation offers a potential solution for type 1 diabetes by restoring insulin secretion. However, challenges remain in long-term graft survival and achieving insulin independence for all patients.

  • Analysis of outcomes following the Edmonton Protocol (steroid-free immunosuppression, increased islet mass).
  • Assessment of 5-year follow-up data in 65 patients.

    • The Edmonton Protocol improved 1-year outcomes compared to earlier methods.
    • Significant improvement in glycemic instability was observed in a portion of patients.
    • Only 7.5% of patients achieved insulin independence at 5 years, indicating functional graft loss.

    Conclusions:

    • Human islet transplantation can reduce severe hypoglycemia and improve glycemic control for 4-5 years in selected patients.
    • Limitations persist, including cell loss, graft rejection, and immunosuppression side effects.
    • The current procedure is not universally suitable for all type 1 diabetes patients.
