A rare adrenal leiomyosarcoma, a giant tumor above the left kidney, was successfully treated with surgery. This case highlights the rarity of primary adrenal leiomyosarcoma, with only 22 reported globally.

Observation:

  • A 78-year-old female presented with fever and vomiting, found to have a giant left adrenal mass.
  • Imaging revealed a 10 cm solid, well-defined mass, initially diagnosed as a non-functioning adrenal tumor.

Findings:

  • Histopathological examination of the resected 11 cm mass confirmed it as leiomyosarcoma.
  • The patient remained disease-free for 20 months post-surgery.

Implications:

  • This case contributes to the limited literature on primary adrenal leiomyosarcoma.
