This study presents a case of silent thyroiditis in a young man experiencing thyrotoxic symptoms and hypokalemic periodic paralysis. Hypothyroidism was later caused by thyroid-stimulation blocking antibodies (TSBAb).

Area of Science:

  • Endocrinology
  • Immunology
  • Thyroidology

Background:

  • Thyrotoxicosis can manifest with hypokalemic periodic paralysis.
  • Silent thyroiditis is an autoimmune thyroid disease characterized by transient thyrotoxicosis followed by hypothyroidism.

Observation:

  • A 24-year-old man presented with thyrotoxic symptoms and hypokalemic periodic paralysis, high thyroid hormone levels, suppressed TSH, and positive TSH-binding inhibitor immunoglobulin (TBII).
  • After one month, the patient became hypothyroid with increased TSH and elevated thyroidal uptake, while TBII remained positive.

Findings:

  • Diagnosis of silent thyroiditis was made based on clinical presentation.
  • Thyroid-stimulation blocking antibodies (TSBAb) were detected during the hypothyroid phase, with no detectable thyroid-stimulating antibodies (TSAb).

Related Experiment Videos

Implications:

  • The findings suggest that TSBAb, in conjunction with TBII activity, can cause hypothyroidism in silent thyroiditis.
  • This case highlights the complex autoimmune mechanisms involved in thyroid dysfunction and the importance of antibody profiling.
