    Background:

    • Investigated metacarpal cortical bone mineral density (BMD) changes in early rheumatoid arthritis (RA) using digital X-ray radiogrammetry.
    • Assessed BMD over 1, 2, and 5 years to quantify bone loss.
    • Identified predictors of bone loss within the RA cohort.

    Discussion:

    • Significant metacarpal BMD loss was observed, reaching -5.6% by year 5.
    • Elevated erythrocyte sedimentation rate (ESR) and anti-cyclic citrullinated peptide (anti-CCP) levels independently predicted bone loss.
    • Patients with severe inflammation and high auto-antibody production exhibited the most substantial bone loss.

    • Early RA is associated with progressive metacarpal bone loss.
    • Inflammatory markers (ESR) and auto-antibodies (anti-CCP) are crucial indicators of bone loss risk.
