This study found that elective percutaneous endoscopic tracheostomy (PET) is a safe and cost-effective procedure for critically ill patients. PET in the Intensive Care Unit (ICU) offers a simple approach for airway management in severe respiratory conditions.

Area of Science:

  • Critical Care Medicine
  • Pulmonology
  • Surgical Procedures

Background:

  • Critically ill patients often require prolonged mechanical ventilation.
  • Tracheostomy is a common procedure to facilitate airway management and weaning from ventilation.
  • Percutaneous endoscopic tracheostomy (PET) offers a minimally invasive alternative.

Purpose of the Study:

  • To evaluate the safety and efficacy of elective percutaneous endoscopic tracheostomy (PET) in critically ill patients.
  • To assess the feasibility of performing PET in the Intensive Care Unit (ICU) setting.

Main Methods:

  • 61 critically ill patients underwent elective PET between June 1988 and August 1989.
  • Procedures involved dilation up to 36 Fr. followed by tracheostomy tube insertion (size 6-10).

Related Experiment Videos

  • Patients' ventilation duration before and after PET was recorded.
  • Main Results:

    • One patient (1.6%) died from cardiac arrest.
    • Four patients (6.6%) required reintubation due to tube obstruction or cuff defects.
    • Two significant infections, one minor bleeding event, and one case of emphysema were observed.
    • Average ventilation duration was 17 days pre-PET and 28 days post-PET.

    Conclusions:

    • Elective percutaneous endoscopic tracheostomy (PET) is a simple and cost-effective procedure for critically ill patients.
    • Performing PET in the ICU is feasible and can be beneficial for airway management.
    • While complications can occur, PET demonstrates a favorable risk-benefit profile in selected patients.
