Background:

  • Fibrofolliculoma is a benign skin neoplasm.
  • Multiple fibrofolliculomas may indicate Birt-Hogg-Dube syndrome.
  • Solitary fibrofolliculoma is rare.

Observation:

  • A solitary fibrofolliculoma on the nose of a 63-year-old woman was examined.
  • The lesion exhibited bizarre multinucleated perifollicular stromal cells positive for factor XIIIa.
  • CD34-positive cells and peripheral CD34-positive spindle cell fascicles were noted.

Findings:

  • The bizarre perifollicular cells are proposed as an 'ancient' feature of fibrofolliculoma.
  • Co-expression of CD34 and factor XIIIa supports the 'ancient' feature hypothesis.
  • Peripheral spindle cell fascicles suggest a pseudosarcomatous proliferation stimulated by factor XIIIa-positive dendrocytes.

Implications:

  • This case expands the understanding of fibrofolliculoma histology.
  • The findings contribute to differentiating benign lesions from sarcomas.
  • Further research may clarify the role of CD34 and factor XIIIa in fibrofolliculoma development.
