Background:

  • Beta-lactam antibiotic allergy diagnosis is standardized.
  • The negative predictive value (NPV) of drug provocation tests (DPTs) requires further establishment.

Purpose of the Study:

  • To evaluate the NPV of DPTs for beta-lactam hypersensitivity.
  • To assess the safety of beta-lactam re-exposure following negative allergy work-ups.

Main Methods:

  • A historical-prospective multicentre cohort study involving 457 patients across four European centers.
  • Patients with suspected beta-lactam allergy underwent DPTs and were followed up for at least six months.
  • Re-exposure to the negatively tested beta-lactam was assessed.

Main Results:

  • 365 patients (79.9%) were followed up; 118 were re-exposed to the beta-lactam.
  • Nine patients (7.6%) experienced non-immediate, non-severe reactions (urticaria, exanthema).
  • The calculated NPV of DPTs was 94.1% (95% CI: 89.8-98.3).

Conclusions:

  • Drug provocation tests for beta-lactams demonstrate a high NPV, though not 100%.
  • False negative results did not lead to severe or life-threatening reactions.
  • These findings support the safe re-prescription of beta-lactams in patients with negative allergy evaluations.
