Summary

Dengue fever can cause upper gastrointestinal bleeding, often due to peptic ulcers. Supportive care and transfusions were effective treatments in most Dengue patients studied.

Area of Science:

  • Infectious Diseases
  • Gastroenterology
  • Virology

Background:

  • Dengue fever is a significant public health concern, particularly in tropical and subtropical regions.
  • Upper gastrointestinal bleeding (UGIB) is a serious complication that can occur in Dengue patients.
  • Understanding the specific gastrointestinal manifestations of Dengue is crucial for effective management.

Purpose of the Study:

  • To investigate the incidence and endoscopic findings of upper gastrointestinal bleeding in Dengue fever patients.
  • To identify risk factors and clinical characteristics associated with UGIB in Dengue.
  • To evaluate the treatment outcomes for Dengue-associated UGIB.

Main Methods:

  • A retrospective study of 198 Dengue fever patients admitted to Kaohsiung Chang Gung Memorial Hospital from 1987 to 1988.

Related Experiment Videos

  • Gastroduodenoscopy was performed on 26 Dengue patients presenting with signs of upper gastrointestinal bleeding.
  • Viral isolation and serological studies were conducted to confirm Dengue type I infection.
  • Main Results:

    • Twenty-six Dengue patients (13.1%) experienced upper gastrointestinal bleeding.
    • Gastric and duodenal ulcers were the most common endoscopic findings, along with hemorrhagic gastritis.
    • Half of the affected patients had a prior history of peptic ulcer symptoms; the other half did not.
    • Dengue infection was identified as a precipitating factor for peptic ulcer bleeding due to hemostatic disturbances.

    Conclusions:

    • Dengue virus infection can precipitate upper gastrointestinal bleeding, particularly peptic ulcer hemorrhage.
    • Endoscopic evaluation is essential for diagnosing the cause of UGIB in Dengue patients.
    • Conservative management, including supportive therapy and blood transfusions, is generally effective, with surgery rarely required.
