Background:

  • Transvaginal oocyte retrieval (TVOR) is a key procedure in assisted reproductive technologies.
  • Intraperitoneal bleeding is a rare but significant complication of TVOR.

Observation:

  • A retrospective review analyzed 10,251 TVOR cycles between 2004 and 2008.
  • 22 patients (0.2%) experienced intraperitoneal bleeding, with 5 severe cases requiring surgical intervention.

Findings:

  • Significant risk factors for bleeding included lower body mass index and a higher number of oocytes retrieved.
  • Severe bleeding was associated with younger age, lower BMI, prior surgery, and moderate ovarian response.
  • Mild bleeding was linked to younger age and high ovarian response.

Implications:

  • Identifying patients at risk for intraperitoneal bleeding can improve TVOR safety.
  • Further research may refine patient selection criteria and monitoring protocols for TVOR.
