Inflammatory mediators and posterior segment involvement in ocular Behcet disease

Background:
- Behcet disease (BD) is a multisystem inflammatory disorder.
- Ocular involvement, particularly posterior uveitis, is a significant cause of visual impairment in BD patients.
Purpose of the Study:
- To investigate the serum levels of key inflammatory mediators.
- To determine the association between these mediators and active posterior ocular inflammation in BD.
Main Methods:
- A prospective clinical trial included 43 patients with ocular BD, divided into active (Group A) and inactive (Group B) posterior segment involvement.
- Serum levels of IL-6, IL-8, TNF-alpha, VEGF, and MDA were quantified using ELISA and spectrophotometry.
- Statistical analysis included independent sample t-tests and Mann-Whitney U-tests to compare groups.
Main Results:
- Patients with active posterior uveitis (Group A) exhibited significantly higher serum levels of IL-6, IL-8, TNF-alpha, VEGF, and MDA compared to those with inactive disease (Group B).
- Specific mean levels in Group A were IL-6 (23.1 pg/mL), IL-8 (161.1 pg/mL), TNF-alpha (30.5 pg/mL), VEGF (455.9 pg/mL), and MDA (10.7 microm).
- Group B showed lower mean levels: IL-6 (12.2 pg/mL), IL-8 (48.4 pg/mL), TNF-alpha (18.6 pg/mL), VEGF (138.0 pg/mL), and MDA (5.1 microm).
Conclusions:
- Increased serum concentrations of IL-6, IL-8, TNF-alpha, VEGF, and MDA are associated with active posterior uveitis in Behcet disease.
- These inflammatory mediators may play a role in the pathogenesis of posterior ocular inflammation in BD.
- Further research could explore therapeutic targeting of these mediators.
