Delayed interval delivery in a triplet pregnancy; a case report
1St. Elisabeth-Hospital, Tilburg, The Netherlands.
Summary
This case report details a triplet pregnancy with an early delivery of one infant at 20 weeks. The remaining two infants were successfully delivered 49 days later using cervical cerclage and tocolysis.
Area of Science:
- Perinatology
- Maternal-Fetal Medicine
- Obstetrics
Background:
- Triplet pregnancies present unique challenges in management and delivery.
- Immature delivery, especially in multifetal gestations, carries significant risks for both mother and neonates.
Observation:
- A triplet pregnancy resulted in the premature delivery of the first infant (triplet A) at 20 weeks gestation.
- A subsequent delay in delivery for the remaining two infants (triplets B and C) was achieved.
Findings:
- Cervical cerclage and tocolysis were employed to successfully prolong the pregnancy by 49 days after the initial delivery.
- This represents the first reported instance of two surviving infants being vaginally delivered after the preceding vaginal delivery of a co-triplet.
Implications:
- This case highlights the potential efficacy of cervical cerclage and tocolysis in managing extremely preterm deliveries in triplet pregnancies.
- It suggests a possible approach for improving outcomes in selected cases of multifetal pregnancies with extremely preterm rupture of membranes or delivery.
