Rare lumbar hernias can cause acute bowel obstruction. Early open repair with mesh insertion is crucial for incarcerated small bowel in spontaneous inferior lumbar hernias to prevent ischemia.

  • The patient did not have a history of surgery around the affected anatomical site.

  • The incarcerated small bowel was treated before the onset of ischemia.

Implications:

  • This case underscores the importance of thorough clinical examination for diagnosing lumbar hernias.
  • A high index of suspicion is vital for early diagnosis and intervention, even without prior surgical history.
  • Prompt surgical repair can prevent complications like bowel ischemia in incarcerated lumbar hernias.
