Summary

Unilateral coronoid hyperplasia was successfully treated with coronoidotomy and physiotherapy, significantly increasing mouth opening. Postoperative X-rays showed good healing of the coronoid process to the mandibular ramus.

Area of Science:

  • Oral and Maxillofacial Surgery
  • Orthodontics
  • Radiology

Background:

  • Coronoid hyperplasia can cause limited mouth opening and coronoid-malar interference.
  • Surgical intervention like coronoidotomy is considered for severe cases.

Observation:

  • A 28-year-old male presented with limited maximum mouth opening (30 mm) due to unilateral coronoid hyperplasia.
  • The patient underwent an intraoral coronoidotomy of the left coronoid process.

Findings:

  • Postoperative physiotherapy with a HU-OS(r) appliance was initiated 9 days after surgery.
  • Maximum mouth opening improved to 43 mm following coronoidotomy and physiotherapy.
  • Radiographic follow-up revealed union of the coronoid process to the mandibular ascending ramus, with some posterior dislocation and inclination.

Implications:

  • Intraoral coronoidotomy combined with prolonged physiotherapy offers a viable treatment for coronoid hyperplasia.
  • This approach achieved satisfactory and stable correction of coronoid-malar interference.
  • The findings highlight the importance of postoperative rehabilitation for optimal functional outcomes.
