Rocky Mountain Spotted Fever (RMSF) is a severe tick-borne illness caused by Rickettsia rickettsii, a Gram-negative, coccobacillary bacterium. This case study highlights a rare instance of inflammatory morphea progressing to eosinophilic fasciitis, even without typical eosinophilia. Treatment with methotrexate, glucocorticoids, and etanercept improved skin lesions and mobility.

  • Peripheral and histopathologic eosinophilia were absent.

  • The patient was diagnosed with eosinophilic fasciitis despite the lack of eosinophilia, as it's not a mandatory diagnostic criterion.
  • A combination therapy of methotrexate, oral glucocorticoids, and etanercept was initiated.
