Summary

Interstitial granulomatous dermatitis with arthritis is a rare condition. This case study shows successful treatment using a combination of potent topical and intralesional glucocorticoids, narrow-band ultraviolet B phototherapy, systemic glucocorticoids, and methotrexate.

Background:

  • Interstitial granulomatous dermatitis (IGD) is a rare inflammatory skin condition.
  • The association between IGD and arthritis, while uncommon, presents unique diagnostic and therapeutic challenges.

Observation:

  • A 54-year-old woman presented with a history of arthritis and long-standing, symmetric, indurated plaques on the thighs and trunk.
  • Histopathologic examination of skin biopsy confirmed interstitial granulomatous dermatitis.

Findings:

  • A diagnosis of interstitial granulomatous dermatitis with arthritis was established.
  • Treatment involved potent topical and intralesional glucocorticoids, narrow-band ultraviolet B (NB-UVB) phototherapy, systemic glucocorticoids, and methotrexate.

Implications:

  • This multimodal therapeutic approach led to significant improvement in both the cutaneous and musculoskeletal manifestations of the disease.
  • The findings suggest that a combination therapy can be effective for managing this complex condition.
