Switching to lighter cigarettes may hinder quitting. Smokers who switched had fewer successful quit attempts and lower recent quitting rates, resulting in a 46% reduction in overall net quitting success.
Area of Science:
- Tobacco Control Research
- Behavioral Science
- Public Health
Background:
- Smokers often switch to "lighter" cigarettes believing they are less harmful or easier to quit.
- This behavior may paradoxically reduce motivation and success in quitting smoking altogether.
Purpose of the Study:
- To investigate factors associated with switching to lighter cigarettes.
- To determine the impact of switching on making a quit attempt and achieving recent quitting, defining "net quitting" as the product of attempts and recent quitting.
Main Methods:
- A 2003 survey of 30,800 ever-smokers assessed cigarette switching history and reasons (harm reduction, quitting, flavor).
- Quit attempts and recent quitting (≥90 days abstinence) were analyzed using multivariable logistic regression.
- Net quitting was calculated as the product of quit attempts and recent quitting.
Main Results:
- 38% of ever-smokers had switched cigarettes.
- Switchers were more likely to attempt quitting (51% vs. 41%) but less likely to have recently quit (9% vs. 17%).
- Switching was associated with a 46% lower odds of net quitting (AOR 0.54), particularly when switching was not motivated by quitting smoking.
Conclusions:
- Switching to lighter cigarettes is associated with significantly lower odds of successful quitting.
- Reasons for switching, especially harm reduction alone, did not show a positive association with quitting outcomes.
- Public health strategies should discourage cigarette switching as a harm reduction or quitting method.
