Idiopathic ventricular tachycardia from aortic cusps is rare but treatable. Successful radiofrequency ablation offers a curative option for patients with incessant ventricular tachycardia.

- Idiopathic ventricular tachycardia (IVT) accounts for 10% of ventricular tachycardia cases, with diverse origins.
- Aortic cusp origin IVT is rare (0.7%), presenting distinct electrocardiographic patterns (LBBB, inferior axis).
- IVT from the aortic sinus cusp requires differentiation from right ventricular outflow tract origins.

- This report details the first successful radiofrequency catheter ablation of IVT originating from the aortic sinus cusp in Mexico.
- The patient presented with incessant ventricular tachycardia, a challenging clinical scenario.

- Radiofrequency catheter ablation achieved successful treatment for aortic cusp IVT.
- The procedure demonstrated high efficacy (>90%) and a low risk of serious complications (<1%).
Implications:
- Aortic cusp IVT should be considered in the differential diagnosis of ventricular tachycardia.
- Radiofrequency catheter ablation is a safe and effective first-line therapy for selected IVT patients.
- This case highlights the feasibility of successful ablation in a previously unreported region and country.
