Background:

  • Polyomavirus-associated nephropathy (PVAN) is a significant cause of kidney transplant dysfunction.
  • The role of plasma cells (PCs) in PVAN pathogenesis remains poorly characterized.

Purpose of the Study:

  • To characterize plasma cells in PVAN kidney biopsies.
  • To correlate plasma cell findings with clinical, morphological, and serological data.

Main Methods:

  • Immunohistochemistry and morphometry were used to analyze plasma cells in 20 PVAN cases.
  • Results were correlated with clinical, morphological, and anti-BK virus serological findings.

Main Results:

  • PC-rich infiltrates (>15% PCs) were found in 50% of cases.
  • IgM-producing PCs were common (70%), with predominance in 50% of cases.
  • IgM PC counts correlated significantly with serum IgM antibodies against BK virus (anti-BKV).

Conclusions:

  • IgM PC presence in biopsies indicates active PVAN infection.
  • PC-rich infiltrates with IgM PCs and negative C4d staining in peritubular capillaries suggest initiating PVAN infection investigation.
