Summary

Subconjunctival fibrosis often causes bleb failure after glaucoma surgery. Administering mitomycin-C 24 hours before THC:YAG laser sclerostomy significantly prolongs bleb duration and lowers intraocular pressure in rabbits.

Area of Science:

  • Ophthalmology
  • Surgical Innovation
  • Glaucoma Research

Background:

  • Subconjunctival fibrosis is a primary cause of bleb failure following glaucoma surgery.
  • Effective inhibition of the wound-healing response is crucial for sustained intraocular pressure (IOP) reduction.

Purpose of the Study:

  • To evaluate the efficacy of subconjunctival mitomycin-C combined with THC:YAG laser sclerostomy in preventing bleb failure.
  • To determine the optimal timing for mitomycin-C administration in relation to the filtering operation.

Main Methods:

  • Forty-eight rabbits underwent THC:YAG laser sclerostomy with a conjunctival wound for energy delivery.
  • Subconjunctival mitomycin-C injections were administered 24 hours before (Group I), 5 days before (Group II), or immediately after (Group III) surgery.

  • Group IV received a saline injection as a control.
  • Main Results:

    • Bleb survival was significantly longer in Group I (30.9 days) compared to Groups II (16.0 days), III (15.5 days), and IV (2.3 days).
    • The IOP lowering effect correlated directly with the duration of bleb formation.
    • Preoperative mitomycin-C administration demonstrated superior outcomes over postoperative administration.

    Conclusions:

    • Subconjunctival mitomycin-C injection 24 hours prior to THC:YAG laser sclerostomy is the most effective strategy for prolonging bleb survival.
    • This approach offers a promising method to improve long-term IOP control in glaucoma patients.
    • Optimizing the timing of antifibrotic agents is key to enhancing surgical outcomes in glaucoma.
