Summary

Epidermal nevus syndrome can cause serious neurologic issues like stroke. Underlying blood vessel abnormalities, not brain overgrowth, may explain these brain lesions in patients.

Background:

  • Epidermal nevus syndrome (ENS) is a rare congenital disorder characterized by the presence of epidermal nevi and abnormalities in various organ systems.
  • Neurologic manifestations in ENS, including stroke and seizures, are increasingly recognized but their underlying mechanisms remain unclear.
  • Previous studies have suggested a link between ENS and central nervous system abnormalities, but specific vascular pathologies have not been consistently identified.

Observation:

  • This study reports a patient with ENS and a right hemispheric infarct.
  • A review of three additional cases with neurologic manifestations consistent with ischemia or hemorrhage was performed.
  • Key vascular abnormalities, such as vessel occlusion or dysplasia, were noted in all reviewed patients.

Findings:

  • The reviewed patients presented with significant vascular abnormalities, including vessel occlusion and blood vessel dysplasia.
  • None of the patients with neurologic manifestations exhibited hemimegalencephaly.
  • The findings suggest a strong association between vascular dysplasia and neurologic lesions in ENS.

Implications:

  • Underlying vascular dysplasia is hypothesized to be the primary cause of neurologic lesions in patients with epidermal nevus syndrome.
  • This hypothesis warrants further investigation to elucidate the precise mechanisms linking vascular abnormalities to neurologic deficits in ENS.
  • Understanding this association may lead to improved diagnostic and therapeutic strategies for neurologic complications in ENS.
